MyMilestone, a secure online service, allows patients to view, print and manage key information after an office visit. Using a secure user name and password, patients have convenient access to test results, communication to staff, or request an appointment from anywhere with Web service.
To request an appointment, visit MyMilestone or call the office with whom you would like to schedule an appointment.
Please arrive early if you have not completed new patient forms. The forms may be completed online at MyMilestone. Or, if you would like, you can complete the forms at home and bring them to your appointment. Thank you.
We see patients by appointment only. Same day appointments are available for both urgent needs such as sudden illness as well as routine needs such as physicals. However these same day appointments are limited and fill up quickly. Please call us before seeking care elsewhere so that we may try to accommodate your needs.
Co-pays are expected to be paid at time of appointment. If a patient does not have their co-pay at time of service they may be asked to reschedule. Patients with outstanding balances are asked to make at least 10% payment on the outstanding balance at time of service or the appointment may need to be cancelled. Please contact their pharmacy seven days prior to needing refill. If you run out on a current prescription, call your pharmacy. This applies to all prescriptions including those that must be written and hand carried to the pharmacy. Please note, there will be times that medications cannot be refilled and you will be asked to schedule an appointment or make arrangements for other testing. Refills are handled during normal business hours. Do not contact the on-call physician for refill requests.
Visit MyMilestone to review test results at your convenience. The Milestone Medical Group team will also notify you. In some cases, you may be asked to schedule an appointment to discuss the results.
Advice & Emergencies. For an emergency call 911 immediately. If you have an urgent need after hours, call the office number and you will reach the answering service. The answering service will page the Milestone Medical Group physician that is on call.
Honoring time commitments allows us to provide you, and all our patients, with the best professional care and service. If you need to cancel your appointment, we require 24-hour notice for a routine scheduled appointment and notification within four hours for illness-related visits. If you missed the appointment, you are considered a “no show.” When a patient has three “no show” appointments within one year, the patient will be dismissed from the practice and asked to seek health care outside of Milestone Medical Group. Please call as soon as you know that you are unable to keep your appointment. Thank you.
