Centura Health Physician Group - Family Medicine

A service of Longmont United Hospital, Centura Health Physician Group - Family Medicine aims to provide you with individualized care that sees you as a part of your family and your community. We focus on the whole body, and understand the interconnectedness of your health, while factoring in the external influences in your life. Our holistic approach intends to provide information on disease prevention for you in addition to diagnosing and treating any illness you may experience. Our family practice is a division of primary care that provides health care for all ages, genders, and for all of the body, throughout your life.

Our family physicians are also connected to specialists when you need them, all coordinated through your primary doctor. Patient-Centered Medical Homes

We are proud to have earned the highest level of NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care. This achievement reflects our dedicated efforts to provide high-value, patient-centered care to our patients.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits

When you choose a Patient-Centered Medical Home, you will receive better, more personal care. Your Centura Health doctor will lead a team that may include nurses, medical assistants, front desk staff, mental health counselors, pharmacists, health educators, and care coordinators. This team will work together to support your needs, monitor your health over time, and coordinate your care. So the most important member of the team is you!

Get involved in your health

Your care team will encourage you to stay healthy by:

  • Setting your own health goals
  • Getting to know your care team and help them get to know you
  • Making your best effort to come to each appointment (or call if you need to reschedule)
  • Sharing your medical history and records from every place where you get care
  • Bringing your insurance card and medications to every visit
  • Signing up and use the patient portal
  • Discussing your specialty care needs so we can help coordinate your care
