Welcome! Here you will find all the information you need for your upcoming appointment. If you have any questions, please call one of our physician offices.

Preparing for Appointments

Getting ready for your appointment
  • Make a list of questions.
  • Make a list of health care providers / facilities that you have visited since last seen at our office.
  • Bring all your medications, in their original containers to your appointment. Include over the counter vitamins and supplements.
  • Bring your insurance card and photo ID.
  • Consider asking a friend or family member to go with you to your appointment.

During your appointment
  • Talk openly with your team.
  • Ask your most important questions first.
  • Use your own words to repeat back the things you have discussed with your care team. This will confirm that all information is clear.
  • Be an active participant. Collaborate with your care team to make goals to keep you healthy.
  • Before you leave, be sure you understand goals and steps to take to reach those goals.

Same Day Appointments

Same day appointments are available. Please call us before seeking care elsewhere so that we may try to accommodate your needs. Same day appointments are limited and fill up quickly, please call us as soon as you can.

Secure On-line Patient Portal

We offer a Patient Portal that allows patients to view, print, and manage key information after an office visit. Using a secure username and password, patients have convenient access to test results, communication to staff, or to request an appointment from anywhere with web service.

Co-pays & Payments

Some insurance plans have co-pays. Please plan to pay your co-pay at the time of appointment. If you can not pay your co-pay at the time of appointment you may be asked to reschedule. If there is an outstanding balance on your account you will be asked to make a payment at the time appointment or the appointment may need to be rescheduled. Please contact our billing office at 866-588-3588 and they will be happy to assist you and answer any billing questions you may have.

Prescription Refills

If you need a refill of medication we prescribed for you, please contact your pharmacy. This applies to all prescriptions including those that must be written and hand carried to the pharmacy. Should your prescription need to be renewed, your pharmacy will contact us and take the necessary steps to provide you with your medication refill. Please note, there will be times that medications cannot be refilled and you will be asked to schedule an appointment or make arrangements for other testing. Refills are handled during normal business hours and we ask that you allow 72 hours for processing. There may be times that our providers will need to see you prior to refilling your medication and we will contact you to make arrangements.


We try to inform you of your test results as soon as they are available and reviewed by your provider. If we have not contacted you with your test results within a week of completion of your test, please call us.

After Hours

Advice & Emergencies. For an emergency call 911 immediately. If you have an urgent need after hours, call the office number for information on reaching the on-call provider.

Cancellation Policy

Honoring time commitments allows us to provide you, and all our patients, with the best professional care and service. We understand that circumstances can occur that make it necessary for you to cancel or reschedule an appointment. Please contact us as soon as you know you will need to miss an appointment. We often have people we can move up in the schedule if we know there will be an opening.
