Centura Health Physician Group

Centura Health Physician Group - Internal Medicine

Centura Health Physician Group - Internal Medicine, a service of Longmont United Hospital, specializes in adult internal medicine and its subspecialties. Our internists handle the broad and comprehensive spectrum of illnesses that affect adults. We are recognized as experts in diagnosis, in treatment of chronic illnesses, and in health promotion and disease prevention. Internists are the “doctor’s doctor,” because we are often called upon to serve as consultants to other physicians to help solve baffling diagnostic problems. Our physicians are equipped to deal with whatever problem an adult patient brings—no matter how common or rare, or how simple or complex.

We take pride in caring for our patients for the duration of life—in the office or clinic, during hospitalization and intensive care, and in nursing homes. When other specialists, such as surgeons or obstetricians, are involved, we coordinate our patients’ care and manage any and all medical problems associated with that care.

Patient-Centered Medical Homes

We are proud to have earned the highest level of NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care. This achievement reflects our dedicated efforts to provide high-value, patient-centered care to our patients.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits

When you choose a Patient-Centered Medical Home, you will receive better, more personal care. Your Centura Health doctor will lead a team that may include nurses, medical assistants, front desk staff, mental health counselors, pharmacists, health educators, and care coordinators. This team will work together to support your needs, monitor your health over time, and coordinate your care. So the most important member of the team is you!

Get involved in your health

Your care team will encourage you to stay healthy by:

  • Setting your own health goals
  • Getting to know your care team and help them get to know you
  • Making your best effort to come to each appointment (or call if you need to reschedule)
  • Sharing your medical history and records from every place where you get care
  • Bringing your insurance card and medications to every visit
  • Signing up and use the patient portal
  • Discussing your specialty care needs so we can help coordinate your care
