Requesting Medical Records at Longmont United Hospital

Longmont United Hospital maintains a medical record for each patient seen in the hospital, emergency room, outpatient, or laboratory setting. The Hospital also has a legal and ethical responsibility to preserve the privacy of the patient’s protected health information. The release of information from a patient medical record requires prior authorization by the patient or their legal representative. Information will not be released, disclosed, or made available for review until this authorization is received.

To request a copy of your medical and/or billing records, you will need to complete an authorization form and provide a valid government issued photo identification. Click here for the Medical Record Release Form in English or en Español. Please complete this form then mail or fax to Medical Records Department.
