Requesting Medical Records
Longmont United Hospital maintains a medical record for each patient seen in the hospital, emergency room, outpatient, or laboratory setting. The Hospital also has a legal and ethical responsibility to preserve the privacy of the patient’s protected health information. The release of information from a patient medical record requires prior authorization by the patient or their legal representative. Information will not be released, disclosed, or made available for review until this authorization is received.
To request a copy of your medical and/or billing records, you will need to complete an authorization form and provide a valid government issued photo identification. Click here for the Medical Record Release Form (En Español). Please complete this form then mail or fax to Medical Records Department.
State Fee Copy Costs
Please note, there is a State Fee for copies of records for personal use of $14.00 for 10 pages or less. Additional pages cost: $0.50 for pages 11-40, and $0.33 for pages 41 and over. Copies for continuation of care will be sent to your Physician at no charge.

Birth Certificates
Longmont United Hospital electronically completes birth certificate information for every infant born at the hospital.
This is done in compliance with the State of Colorado requirements. Once the information has been entered, it is automatically transmitted to the State. Copies of Birth Certificates are available from:
