Advance Directives

You have the right to make decisions about your health care. Making such decisions ahead of time is called ADVANCE DIRECTIVES. The three kinds recognized in Colorado are:

  • The Living Will: This allows you to express your wishes in case of a terminal illness or an untreatable condition.
  • The Durable Power of Attorney for Medical Care: this allows a person you name to make decisions for you if you are unable to make them.
  • Do Not Resuscitate (NDR): this allows you to refuse resuscitation if you stop breathing or your heart stops.

If you have one or all of these papers, they will guide decisions if you are unable to speak for yourself. Please give us a copy of your Advance Directives.

Your Advance Directives can name a person to speak for you in case you cannot make decisions about your care. It you have not named a person to speak for you, family or friends may choose one. Or we will find a person who can make medical decisions for you.

Longmont United Hospital staff respect your right to have Advance Directives. This includes your choice to accept or refuse medical treatment. It is our policy to honor your choices and teach you about these rights.

If you would like more information on Advance Directives, call Volunteer Services at 303.651.5205.
