Our Heart Care Team of cardiologists, nurses and dedicated staff are available 24 hours a day, seven days a week to provide both scheduled and emergency heart care in a compassionate and personalized environment.


Cardiologists

Our physicians offer comprehensive support for the diagnosis and treatment of cardiac patients. Their expertise along with Longmont United Hospital facilities, staff and services offer individuals access to advanced medical services, comprehensive rehabilitation programs, educational classes and support groups for inpatient and outpatients.

Cardiology Nurses

Cardiology nurses are Progressive Care Certified Nurses (PCCN). Our PCCN nurses work with a multidisciplinary team to develop a comprehensive plan of care. They also serve as patient educators that encourage patients to learn about heart disease, prevention, and health maintenance.

We also have cardiac rehabilitation nurses who assist you in developing an exercise and education plan to get you back to work or to the activities you enjoy. As a team, all of our health care professionals work to provide optimal care for you.

Cardiac Catheterization Lab Team

From Cardiovascular Technologists to Critical Care Nurses, our Cardiac Cath Lab Team provides 24-hour coverage to any emergency heart care without exception. Our commitment to the community is to provide quick coronary artery intervention, if needed, to heart attack patients arriving in our Emergency Department. When a heart emergency arises, we will be there for you before, during, and after cardiac treatment.

For cardiac catheterization outpatients, we continue your care after you are discharged from the hospital. A cardiac cath lab team member will call you within 48 hours to help with questions you may have regarding your post-cardiac procedure care, to ensure that you are comfortable, and to check if you have made your follow-up appointment with your physician.

Care Coordinator Specialists

Care Coordination is a coordinated effort between you, your family and the Health Care Team to develop an appropriate plan of care to meet your needs during and after hospitalization. The Care Coordination staff consists of Licensed Clinical Social Workers and Care Coordination RNs. Care Coordination is offered so that if needed, home care nurses, hospital equipment, Meals on Wheels, or other services will be there when you go home. The staff can provide information and support about skilled nursing facilities, financial resources, support groups and information about other long-term care services.
