Transitional Care...Where Extraordinary Care Fosters Patient Independence
The Transitional Care Unit (TCU) is a fifteen bed skilled nursing unit at Longmont United Hospital which uses an interdisciplinary approach to patient care. Emphasis is on pain management, IV therapy, wound management, end of life care and therapy services. Twenty-four hour nursing care and seven days per week therapy services are provided. Our goal is to assist the patient to become as independent as possible with discharge to the safest environment after their stay in TCU.
